When the recipe calls for "1 cup sifted flour", it means you measure the sifted flour to 1 cup. 
With the comma, there are two instructions: measuring and then sifting. 
With no comma, there is only one instruction: measure 1 cup of SIFTED flour. (Okay, that is two instructions if you count the sifting first part.)
